| Our pharmacists
provide anticoagulation monitoring via the CoaguChek™ monitor
for several of our patients who take warfarin (Coumadin®)*. This
simple, convenient method provides accurate results in only 2 minutes
compared with hours or even days using conventional laboratories.
It uses a small sample of blood from a fingerstick rather than a more
painful sample of blood from a vein.
A progress note is completed by one of our clinical staff pharmacists
at each visit describing patient problems or symptoms related to therapy,
new or discontinued medications, compliance issues, an assessment of the
patient’s status, and recommendations for dosage change and/or future
follow-up. This information is then faxed to the physician for approval,
and once accepted, the follow-up plan is put in place.
*This service can only be performed upon physician referral.
